Changes and clinical significance of serum troponin Ⅰ and ischemia modified albumin in patients with sepsis

摘要: 目的:探讨脓毒症患者血清心肌肌钙蛋白Ⅰ(cTnI)及缺血修饰白蛋白(IMA)的变化及临床意义。方法:选取我院2018年1月—2019年12月期间共收治的100例脓毒症患者,患者入院后均进行床边心脏超声,并根据患者入院24 h内的左心室射分数(LVEF),分为LVEF<50%组(57例)和LVEF≥50%组(43例),另取抽取同期我院体检的健康者50例为对照组,比较3组患者的cTnI和IMA指标表达情况。并根据患者入院28 d是否存活与死亡分为存活组(n=37)和死亡组(n=63),对比存活组与死亡组患者各指标水平,并分析cTnI和IMA对脓毒症患者预后的影响。结果:3组患者cTnI和IMA指标对比差异有统计学意义,LVEF<50%组明显高于其他两组患者,差异具有统计学意义(P<0.05);存活组与死亡组患者性别、年龄、BMI、合并多器官功能衰竭、白细胞计数、肌酐对比差异无统计学意义(P>0.05),两组患者cTnI和IMA、Lac对比差异有统计学意义(P<0.05);logistic回归分析显示cTnI和IMA为脓毒症患者的预后不良影响因素(P<0.05)。结论:脓毒症患者cTnI和IMA指标高于健康者,心肌损伤患者的cTnI和IMA指标数值更高,cTnI和IMA升高可以影响脓毒症患者的预后情况。Abstract: Objective: To investigate the changes and clinical significance of serum cardiac troponin Ⅰ(cTnI) and ischemia modified albumin(IMA) in patients with sepsis. Methods: A total of 100 patients with sepsis admitted to our hospital from January 2018 to December 2019 were selected. All patients underwent bedside echocardiography after admission, and were divided into LVEF<50% group(57 cases) and LVEF≥50% group(43 cases) according to their left ventricular ejection fraction(LVEF) within 24 hours of admission. In addition, 50 healthy people who were taken for physical examination in our hospital during the same period were selected as the control group to compare the expression of cTnI and IMA in the three groups. The patients were divided into survival group(n=37) and death group(n=63) according to whether they were alive or dead 28 days after admission. The levels of each index of patients in the survival group and the death group were compared, and the effects of cTnI and IMA on the prognosis of patients with sepsis were evaluated. Results: The cTnI and IMA indicators of the three groups were significantly different. The LVEF<50% group was significantly higher than the other two groups, and the difference was statistically significant(P<0.05); There was no significant difference in gender, age, BMI, and multiple in organ failure, white blood cell count, and creatinine between the survival group and the death group(P>0.05). The comparison of cTnI, IMA and Lac between the two groups was significantly different(P<0.05); Logistic regression analysis showed that cTnI and IMA were poor prognosis factors in patients with sepsis(P<0.05). Conclusion: The cTnI and IMA indexes of sepsis patients are higher than those of healthy people. The cTnI and IMA indexes of patients with myocardial injury are higher. The increase of cTnI and IMA can affect the prognosis of patients with sepsis.
