Predictive value of dynamic serum procalcitonin monitoring in prognosis assessment of patients with craniocerebral injury

摘要: 目的:探讨颅脑损伤患者血清降钙素原动态变化对患者预后评估的价值。方法:采用回顾性分析方法,选取颅脑损伤患者120例,根据30 d是否存活分为存活组与死亡组。在入院第1、3、5、7天均抽血检测降钙素原水平,并在当天进行急性生理学与慢性健康状况评分系统Ⅱ(APACHE-Ⅱ)、序贯器官衰竭(SOFA)评分,用受试者工作(receiver operating characteristic,ROC)特征曲线下面积分析颅脑损伤患者血清降钙素原变化对预后的评估效能。结果:两组患者第1天血清降钙素原水平、APACHE-Ⅱ与SOFA评分均显示有明显差异(P<0.05);存活组从第3天开始血清降钙素原水平便明显下降,死亡组变化不明显,与当天APACHE-Ⅱ与SOFA评分均呈正相关性(P<0.05)。受试者工作曲线显示,患者第5天血清降钙素原水平对评估患者30 d预后有重要价值。结论:动态监测颅脑损伤患者血清降钙素原对评估患者30 d预后有重要临床意义。

Abstract: Objective:To explore the value of the dynamic changes of serum procalcitonin in patients with craniocerebral injury to evaluate the prognosis. Method:Retrospective analysis was used to select 120 patients with craniocerebral injury,and the survival group and death group were divided according to whether the 30 days survived. In the 1st,3rd,5th and 7th days of admission,the serum levels of procalcitonin were measured,and the acute physiology and chronic health status scoring system Ⅱ (APACHE-Ⅱ),sequential organ failure (SOFA) scores were performed on the same day,with the participants (receiver operating characteristic,ROC) The evaluation of the prognostic value of serum procalcitonin changes in patients with craniocerebral injury was analyzed under the characteristic curve. Result:Serum procalcitonin levels were found in the 1st day of the two groups of patients. The APACHE-Ⅱ and sofa scores showed significant differences (P<0.05),and the survival group had a significant decrease in serum procalcitonin levels in the 3rd day,the death group was not obvious,and the same day apache-Ⅱ and sofa scores were positively correlated (P<0.05). The working curve of the subjects showed that the serum procalcitonin level in the 5th day of the patients had important value in evaluating the prognosis of 30 days.Conclusion:Dynamic monitoring of serum procalcitonin in patients with craniocerebral injury has important clinical significance in the evaluation of 30-day prognosis.-
