The value of mean platelet volume in patients with respiratory failure due to acute exacerbation of chronic obstructive pulmonary disease

  • 目的:探索平均血小板体积(MPV)对慢性阻塞性肺疾病(COPD)急性加重期呼吸衰竭患者的意义。方法:选取2015-04-01-2016-11-01期间就诊于我院急诊科的COPD急性加重期呼吸衰竭患者92例,其中Ⅰ型呼吸衰竭51例,Ⅱ型呼吸衰竭41例。所有患者在入院后立即查血常规、C反应蛋白(CRP)、血气分析。同时有40名健康体检者作为对照组,年龄、性别、体重指数(BMI)、吸烟史与研究组匹配。比较各组MPV、血常规、血气等各参数关系。结果:Ⅰ型呼吸衰竭组MPV (10.6±1.3)及Ⅱ型呼吸衰竭组MPV (10.1±1.6)显著高于对照组MPV (8.9±0.8);MPV与PO2有明显相关性(r=0.440,P<0.01),MPV与乳酸有明显相关性(r=0.430,P<0.01);MPV与白细胞计数(WBC)有弱相关性(r=0.218,P<0.05),MPV与中性细胞百分比、CRP、PCO2、PH值、血红蛋白、红细胞压积及血小板计数均无明显相关性。结论:在COPD急性加重期呼吸衰竭患者中,MPV升高可以反映患者缺氧及组织低灌注情况,MPV与炎症指标相关性不显著。
